How to refer

Patients can be referred to Dr. Steve Tremblay using the online form here or by downloading and printing a referral form. All referrals will be dealt with quickly.

We do ask that any relevant radiographs are posted following the online referral process or included with the paper form. All information provided to the clinic is treated in the strictest confidence.

The fees page can be used as a guide to likely costs for patients.
